CARROLL COUNTY HMIS PRIVACY NOTICE

The information you provide to this agency will be aggregated into the Carroll County’s Homeless 
Management Information System (HMIS) administered by the Carroll County Government’s Department of 
Citizen Services. Entering data into a HMIS is required for all homeless service providers receiving funding 
from the Department of Housing and Urban Development (HUD). 
Carroll County’s HMIS is a countywide database compiling data from homeless service providers in Carroll 
County. Your personal information in the HMIS is only accessible to designated staff at the Department of
Citizen Services and by agencies with an approved Data Sharing Agreement outlining their responsibilities 
for managing data privacy, security, and confidentiality. Carroll County’s HMIS has Data Sharing Agreements with the following agencies:


• Human Services Programs of Carroll County, Inc. (HSP)
• Carroll County Health Department (CCHD)
• Maryland Statewide Homeless Services Data Warehouse (MSHDW) – See the MSHDW HMIS Privacy

Client information is used by the agencies listed above to enhance service delivery and data quality, to 
provide service coordination to clients, and to evaluate service delivery for equity and system improvement. 
Data Protection and Risks:
Your data is protected by legal agreements signed by agencies with access to Carroll County’s HMIS and by 
electronic encryption of your personal information. Per HMIS regulation, information may also be disclosed 
to avert serious threats to health or safety and to provide evidence of abuse/neglect or domestic violence.

information collected in Carroll County’s HMIS or if there is an injury to you related to the collection of 
information in Carroll County’s HMIS. 


Your Rights:
1. You have the right to decide whether to share any personal information.
2. You have the right to decide whether or not to share your personal information with any agency listed 
above. 
3. You have a right to request any information about yourself entered in Carroll County’s HMIS.
4. You will receive the same services whether or not you share personal information.
5. You have a right to request an updated Carroll County HMIS Privacy Notice at any time.

This policy may be amended at any time, and amendments may affect information obtained before the date 
of the change. An amendment to the privacy notice regarding use or disclosure will be effective concerning 
data entered before the amendment unless otherwise stated.
